Social Prescribing

Many things can affect our health that aren't medical conditions. Feeling lonely, struggling with money worries, caring responsibilities, housing problems, bereavement, or a lack of confidence can all have a significant impact on your wellbeing.

A Social Prescribing Link Worker can help you identify what matters most to you and connect you with local services, support groups and community activities that may help improve your quality of life.

Social Prescribing is a free service available through your GP practice.

Rather than focusing on medical treatment, a Social Prescribing Link Worker looks at the wider factors affecting your health and wellbeing. They will take the time to listen, understand your situation and help you access practical support within your local community.

Some people only need a single appointment and some advice or signposting. Others benefit from ongoing support and regular check-ins while they build confidence and access the help they need.

We may be able to support you with:

  • Social isolation and loneliness
  • Bereavement and loss
  • Carer support
  • Housing concerns
  • Debt, financial worries and welfare benefits
  • Employment, training and volunteering opportunities
  • Confidence and self-esteem
  • Community activities and support groups
  • Autism and ADHD support
  • Lifestyle and wellbeing support
  • Accessing local charities and voluntary organisations

Your Social Prescribing Link Worker will:

  • Take time to listen and understand your situation
  • Discuss what is important to you
  • Help you identify goals that matter to you
  • Provide information, advice and signposting
  • Support you to access local services and community groups
  • Help you build confidence and independence

Appointments are confidential and focused on your individual needs.

Mental health support

If you are struggling with your mental health, support is available. Your Social Prescribing Link Worker can help signpost you to appropriate services and support groups.

If you are experiencing a mental health crisis or feel that you may harm yourself, contact NHS 111 and select the mental health option, or call 999 in an emergency.

Bereavement Support Group

Our Primary Care Network runs a Bereavement Support Group which provides a welcoming and supportive environment for people who have experienced the loss of a loved one.

The group offers an opportunity to meet others who understand what you are going through and share experiences in a safe and supportive setting.

Please speak to your practice or Social Prescribing Link Worker for more information.

You can be referred by:

  • A GP
  • Practice nurse
  • Healthcare assistant
  • Reception or care navigation team
  • Other members of the practice team

You can also ask a member of the practice team if you feel Social Prescribing support may be helpful for you.

Remember

You do not need to be unwell to benefit from Social Prescribing.
