What are Primary Care Networks?
Working together to provide better care for our communities
Central and West Warrington Primary Care Network (PCN) brings together seven GP practices serving approximately 68,000 patients across Warrington. By working together, we can provide a wider range of services, share expertise and improve access to care for our patients.
Our member practices are:

A Primary Care Network is a group of neighbouring GP practices that work together with other health and care professionals to provide joined-up care closer to home.
By sharing resources and expertise, Primary Care Networks help patients access a wider range of services and healthcare professionals without always needing to see a GP.
Being part of a Primary Care Network means you may be offered appointments with a range of healthcare professionals who work alongside your GP practice.
These may include:
- Clinical Pharmacists
- Pharmacy Technicians
- First Contact Physiotherapists
- Social Prescribing Link Workers
- Care Co-ordinators
- Health and Wellbeing Coaches
- Mental Health Practitioners
- Paramedics
These professionals have specialist skills and can often provide the right support more quickly, helping you receive the most appropriate care for your needs.
As a Primary Care Network, we work together to provide services that support the health and wellbeing of our local population, including:
- Social Prescribing
- Health and Wellbeing Coaching
- Care Co-ordination
- Enhanced Access appointments
- Long-term condition support
- Preventative healthcare initiatives
- Community wellbeing projects
- Research opportunities
We continually review local health needs to develop services that benefit our patients and communities.
To provide safe, effective and sustainable healthcare for our member practice patient population in partnership with other local healthcare providers.
Healthcare is about much more than treating illness. We work closely with local organisations, voluntary groups, community services and Warrington Borough Council to help people access the support they need to stay healthy and well.
This includes support with:
- Mental wellbeing
- Loneliness and social isolation
- Healthy lifestyles
- Carer support
- Long-term conditions
- Community activities and support groups
We are proud to be a research-active Primary Care Network.
Research helps us improve healthcare, develop better treatments and provide patients with opportunities to contribute to future NHS services. Patients may be invited to hear about research studies that are relevant to them, but participation is always voluntary.
