Our First Contact Physiotherapists can be seen for the following problems:

  • All soft tissue injuries, sprains, strains or sports injuries
  • Arthritis – any joint
  • Possible problems with muscles, ligaments, tendons or bone, eg tennis elbow, carpal tunnel syndrome, ankle sprains
  • Spinal pain including lower back pain, mid-back pain and neck pain
  • Spinal-related pain in arms or legs, including nerve symptoms, eg pins and needles or numbness
  • Changes to walking
  • Post-orthopaedic surgery

The physiotherapist will:

  • Assess you and diagnose what’s happening
  • Give expert advice on how best to manage your condition
  • Refer you on to specialist services if necessary

Care coordinators are one of several new roles that support the NHS’s commitment to improve health through personalised care.  Our Care Co-Ordinators are currently focusing on the Covid-19 vaccination programme both in terms of contacting patients to arrange an appointment and being at the vaccine centre during clinics.

Social prescribing is a key component of Universal Personalised Care.

Social prescribing is a way for local agencies to refer people to a link worker. Link workers give people time, focusing on ‘what matters to me’ and taking a holistic approach to people’s health and wellbeing. They connect people to community groups and statutory services for practical and emotional support.

Social prescribing works for a wide range of people, including people:

  • with one or more long-term conditions
  • who need support with their mental health
  • who are lonely or isolated
  • who have complex social needs which affect their wellbeing.

Social prescribing complements other approaches, such as active signposting.  This is a ‘light touch’ approach where existing staff in local agencies provide information to signpost people to services, using local knowledge and resource directories. Active signposting works best for people who are confident and skilled enough to find their own way to services after a brief intervention.

Our Health Coach sees patients age 16+, diagnosed with or at risk of developing conditions such as Obesity, Diabetes, Hypertension, High Cholesterol, Fatty Liver, Sleep apnoea, Anxiety or Depression.

Patients referred into the service should be ready to make changes to their lifestyle and be willing to engage with our Health Coach to intially complete a 12 week health plan.  Lifestyle changes could include:

  • Weight Loss
  • Diet
  • Exercise
  • Sleep
  • Stopping Smoking
  • Reducing alcohol consumption
  • Reducing stress/improving mood

Clinical Pharmacists are highly qualified experts in medicines and can help people in a range of ways. This includes carrying out structured medication reviews for patients with ongoing health problems and improving patient safety, outcomes and value through a person-centred approach.
